Fildena contains sildenafil, which is a phosphodiesterase type 5 (PDE5) inhibitor. While it is commonly prescribed for erectile dysfunction, it can also help in pulmonary arterial hypertension (PAH). Here’s why: Mechanism of Action in Pulmonary Hypertension: In pulmonary arteries, PDE5 breaks down cyclic GMP (cGMP). cGMP normally causes relaxation of smooth muscle in the blood vessels. By inhibiting PDE5, sildenafil increases cGMP levels, leading to dilation of pulmonary arteries. FILDENA reduces pulmonary vascular resistance, making it easier for the right side of the heart to pump blood through the lungs. Effect on Symptoms: Reduced resistance in the pulmonary arteries lowers pulmonary arterial pressure. Patients often experience improved exercise tolerance and less shortness of breath. Clinical Use: Sildenafil is actually FDA-approved for PAH under the brand name Revatio. The doses used for PAH are usually lower and more frequent than those for erectile dysfunction. So, while Fildena is primarily marketed for erectile dysfunction, the same PDE5 inhibition mechanism can benefit pulmonary hypertension.
